Initiative to provide electronic information to support management of chronic care

Executive Summary:  The Institute of Medicine & Public Health of New Jersey aimed to improve quality of care through establishing claims-based chronic disease registries for small to mid-sized practices.  Funding from The Physicians Foundation supported the Institute of Medicine & Public Health of New Jersey in recruiting data and vendor partners and physician practices. In addition, the grant assisted the project staff in developing recruitment tools and a website.  Over the course of the project, a total of 72 physicians treated 63,773 patients representing data from two practice management systems.
