Virtual, in-person 12-month course

Required Commitments
The program runs for 12 months, from September 2025 to September 2026, and includes:

Virtual monthly instructional sessions on Saturdays from 9-5 p.m. ET from November 2025 – March 2026 and April 2026 – August 2026. |

Requirements
Apply for through the Foundation’s online application system here. Applications must be completed and submitted with all required responses and attachments to be considered. Once you are in the online application system, please log-in or create a new account. Then, use the access code “leadership” to access the application.
Physicians (M.D. or D.O.) in good standing.

How to Apply
Create New Account All applicants must complete a one-time registration. To register, you will need to click on “Create New Account” on the application portal home page and supply your contact information and create a password. Where you are asked for an Organization Name and an Executive Officer name, enter your own name. For the EIN, enter 12-1234567. (If you have used the online system previously to submit an application, you do not need to register again. You can simply log in.)
