Comprehensive program to assist primary care physicians who have already adopted electronic health records in achieving NCQA recognition as patient-centered medical homes

 

Executive Summary: The grantee developed a webinar series on PCMH which provides an overview of the NCQA application process, “do’s and don’ts” and template materials such as policies and procedures, patient surveys and data tables.  Connecticut State Medical Society Foundation collaborated with Qualadigm and developed individual practice based guidance to achieve PCMH recognition.

Among the 19 participating practices, participation in the webinar series was 95 percent. While participation in the Open Line support was only 33 percent, this resource was available to practices that needed additional support or had specific questions. All 19 practices completed their NCQA application for PCMH recognition. To date, 10 practices have been recognized as meeting NCQA Level 3; three practices achieved Level 1 recognition; and six practices have pending applications with NCQA.
