Chronic Care Management – Incorporating Social Drivers of Health
The Foundation of the Pennsylvania Medical Society (FPAMED) is a non-profit 501(c)(3) affiliate of the Pennsylvania Medical Society (PAMED). The Foundation serves as the philanthropic partner for PAMED and advances the collective goals to sustain the future of medicine in Pennsylvania by providing programs that support medical education, physician health, and excellence in practice. The foundation will serve as the 501-c-3 conduit for PAMED to serve as the primary organization to plan, implement and evaluate the Chronic Care Management – Incorporating Social Drivers of Health Project.
The newly approved PAMED strategic plan is centered around helping physicians to identify and address social drivers of health within their practices in all practice settings. In appreciation of practices which have 100% membership, PAMED hosts a monthly “frontline” call to offer expertise from staff regarding emerging topics. The agenda for frontline calls are physician driven as they are asked to identify topics for education and/or discussion that are of importance to their practice setting. It will promote this opportunity to the frontline groups and offer up to twenty (20) AMA PRA Category 1 credits, free of charge, for each physician participating in the project.
