Empower Care: Social Drivers of Health Initiative
The Inland Empire Health Information Organization (IEHIO) is a nonprofit founded in 2010, linked to the Riverside County Medical Association (RCMA), which has represented over 2,200 physicians since 1893. IEHIO’s mission is to break down barriers to healthcare access & ensure equitable service delivery by connecting people with data. It focuses on improving patient care and addressing psychosocial determinants of health through data sharing.
This project aims to integrate Drivers of Health (DOH) screening and resource navigation into patient care processes for up to 10 solo/small group medical offices in Riverside County, CA. Inland Empire Health Information Organization (IEHIO) will leverage its existing tool, ConnectIE, to support these medical practices in connecting patients with essential social services, improving patient outcomes, and aligning with state policies on DOH.
