Initiative to improve care for patients with chronic diseases through health information technology

Executive Summary: The Middlesex Professional Services Foundation carried out a clinical integration project to facilitate communication and sharing of health information among physicians, hospitals, labs and pharmacies to improve efficiency and effectiveness of patient care. Middlesex Professional Services Foundation worked with its network of physicians to implement and utilize an online registry in their practices in order to demonstrate its capacity for managing patient populations with chronic illnesses and to participate in pay for performance initiatives. In particular, efforts were focused on collaborating and engaging with payers to better understand their needs, objectives and concerns. Funding from The Physicians Foundation supported the Middlesex Professional Services Foundation in implementing the improvement project and documenting the barriers physicians face in the adoption of health information technology.
