Methodist Health System is a 130+ year old integrated health network with 4 hospitals, 30+ clinics, and 1,175 providers serving Nebraska and western Iowa. Their mission centers on physician-led, patient-centered care guided by five core values: patient-centered care, respect/integrity, excellence, teamwork, and community service.
Methodist Health System is expanding a successful CHW pilot program to better address DOH for at-risk patients. The project integrates CHWs into ambulatory care settings, the emergency department, and community clinics to screen patients for DOH barriers, then connect them to resources through community partnerships.
