A Model to Improve the Cardiometabolic Health Profiles of At-Risk Chicago Communities
The project will be carried out by the Cardiometabolic Health Initiative (CHI), a unique community service organization founded, led, and operated by Rush University medical students. CHI is an interdisciplinary organization providing mobile, point-of-care cardiometabolic screenings and health coaching at community hubs across Chicago, IL.
The Cardiometabolic Health Initiative (CHI) will use grant funds to conduct monthly screenings for 30 to 90 patients at food pantries, assessing cardiometabolic health, providing personalized health counseling, and tracking improvements over six to twelve months through follow-up screenings and ongoing coaching for those at higher cardiovascular risk.
