Washington State Education & Research Foundation

Executive Summary:  The Washington State Education & Research Foundation sought to provide information and tools to help primary care physicians in small and medium-sized practices to improve the quality of chronic care. The project involved the recruitment of physicians and the collection and analysis of clinical data to determine best practices for delivery and management of chronic care. Funding from The Physicians Foundation supported the Washington State Education & Research Foundation in establishing the organization as a state leader in quality improvement efforts. The grantee was able to use the experience gained through the project to initiate a related effort with the Puget Sound Health Alliance and the Washington Academy of Family Physicians to assist practices in quality improvement.
