Earlier this year, Olivia Mannion had one thing on her to-do list: schedule a six-month follow-up scan. She’s been in remission from Stage IV Hodgkin lymphoma for nearly four years, and while MRIs are always nerve-wracking, booking one was supposed to be the easy part.

My patient Claire and I were at odds. She was 44 years old and came to my clinic for a second opinion. The diagnosis was stage 3 rectal cancer. It started with blood in her stool and quickly turned to weight loss and abdominal pain. I agreed with her previous doctors’ recommendations of surgery, radiation, and chemotherapy. She asked about mistletoe, blue scorpion venom, and infrared lamp therapy.

The way physicians respond to a misinformed patient directly affects clinical outcomes and retention. Communication experts say if you come across as dismissive or paternalistic, the patient is likely to withdraw. You may never see them again.

When Dr. Cornelia Griggs, a pediatric surgeon at Massachusetts General Hospital, was in training, she got lots of advice: Be careful what specialty you go into. Consider what life is going to be like when you’re a parent. Her husband — who is also a surgeon — “didn’t get that advice,” says Griggs, 43. “ Our profession and how we train was structured and designed in an era where it was mostly men going through residency. They mostly had partners who could follow them wherever they landed in the match, and the makeup of our profession looks very different now.”

The shortage of physicians in the United States isn’t a new phenomenon. Unfortunately, it is also one that isn’t expected to disappear anytime soon.

Growing up with a cardiologist dad, Anna always knew she wanted to become a doctor. She had watched her father spend decades building trust with his patients so he could care for them, and she wanted to have that kind of impact.

Nominations are open for an award and grant proposal that aim to connect doctors with their patients’ social drivers of health. Recently, an elderly patient at my practice unexpectedly began to miss her monthly appointments. I was alarmed. The patient has atrial fibrillation—a condition characterized by irregular heart rhythms and an increased risk of stroke. The condition demands consistent and timely medical care. For elderly patients like the one in our care, these appointments are not just check-ups. They are vital for adjusting medications, monitoring heart health, and preventing serious complications like blood clots.

Most physicians and residents report feelings of burnout, while stigma about receiving mental health care remains prevalent, according to a new report from The Physicians Foundation.

According to a survey of more than 1,700 physicians, residents, and medical students, burnout is tied to multiple factors, including mental health stigma in the medical field as well as how private equity acquisitions are changing healthcare.

Twice a week, Boston-area psychiatrist Elissa Ely volunteers at a US anonymous help line for physicians in crisis. The calls she takes are often from people in deep distress — physicians having panic attacks, abusing substances or alcohol, facing divorce or alienation from family and friends. She’s known that medicine was her calling since she was four years old, when she instinctively reset her sister’s dislocated finger (please don’t try it at home, she warns). Caine is one of several parents who followed Dr. Heather Decker to her new location on the outskirts of Minneapolis, an hour and a half away. Many couldn’t get appointments with swamped nearby doctors for months.

This is an extraordinarily difficult time to be a physician. As the leaders of state medical societies and board members of The Physicians Foundation, we represent primary care physicians and specialists across the country, in blue and red states. We’ve witnessed the Covid-19 crisis cost hundreds of thousands of lives and endanger many of our colleagues.

Two years ago, Dr. Kelly McGregory opened her own pediatric practice just outside Minneapolis, where she could spend as much time as she wanted with patients and parents could get all of their questions answered.

On April 3, anesthesiologist ­Michael Peck worked a 12-hour hospital shift intubating critically ill covid-19 patients about to go on ventilators. Despite being gowned, gloved and masked, he was terrified. “I’ve never been that scared,” he recalls. “When it was over, I said to myself, ‘This is crazy. I’m done.’ ”
