Initiative to improve care for patients with chronic conditions in rural Maine

The Blue Hill Memorial Hospital aimed to improve chronic disease prevention and management through practice improvement initiatives. Funding from The Physicians Foundation enabled the project staff at Blue Hill Memorial Hospital to address the need for evidence-based best practice, improve management of chronic care and strengthen community-based health services. After completion of the grant, the Blue Hill Memorial Hospital reported that the project successfully restructured practice care to ensure patients received needed care on time and according to guidelines. Blue Hill Memorial Hospital developed several additional initiatives to continue the work done by the grant and generally improve patient care at Physician Practice Connections (PPC) practices, including eventual implementation of an electronic medical record system, ongoing healthcare improvement teams at the practice and an expansion of the nurse program.
