Initiative to improve clinical office practices and outcomes
Executive Summary: The Medical Care Development undertook an initiative to create a quality improvement structure to support practice change and evidence-based medicine by organizing regional collaboratives within five major physician-hospital organizations. Funding for The Physicians Foundation supported the Medical Care Development staff in developing an assessment tool for evaluating practice readiness for implementation of a chronic disease registry and providing education and training for quality advisors and team leaders. The project implemented chronic disease registries for quality improvements in 77 practices and over 100 practices initiated specific quality improvement activities. The project was successful in establishing a regional improvement network in Maine, which continues to coordinate programs and activities that support practices seeking quality improvements. An active leadership group meets monthly to coordinate programs and plan for the future.
